Administration of Medication Forms
The following forms are necessary for administering medication during school hours. There are three forms noted. Please select the form applicable and have it completed by you, the parent/guardian and the medical provider. Return the completed form with the medication to the Nurse.
Medical providers' forms may be substituted for school forms provided all information is included.

Emergency Medication Form
If your child has an anaphylactic reaction to foods or other allergens or the potential for anaphylaxis, please use the "Emergency Action Plan" form
Please attach a current picture of your child to the form.

Asthma Action Plan is not available on this site. Please contact the School Nurse for this form required for children who receive asthma medication.
