Physical Education Exemptions

What to do if an out of school activity results in injury or illness limits physical activity.....
A visit to the Health Care Provider or Emergency Room/Urgent Care Center indicates physical education limitations.....
Oh..no visit to a Health Care Professional is made and my child may not participate in physical activities......

1. The Health Office receives all documents from parents/guardians related to
modifications in the child’s activities and notifies the appropriate staff/teacher.
2. When the child is wearing any type of apparatus e.g. splint, brace, cast, sling,
immobilizer, etc., they are not permitted to participate in physical education
and other recess activities until removed.
3. When a visit is made to a health care provider please obtain a
note with a diagnosis, length of time activity is to be limited,
approximate return date if known. Emergency Room and Urgent Care
Centers do write this note.
4. If no Health Care Provider is seen or a note is not obtained, a note from the
parent with the same information is submitted. When no documentation is
received, the documentation is provided by the Health Office on a temporary
basis and the parent/guardian is contacted.
